  • Tax Identification Information: Enter your entity's tax identification number, if applicable.   


Part III: Claim of Tax Treaty Benefits


  • Indicate the treaty: Specify the tax treaty under which you are claiming benefits.

  • Enter the name of the foreign country: Provide the name of the country with which the treaty is in effect.

  • Enter the article number of the treaty: Indicate the specific article of the treaty that you are relying on.


Part XXV: Active NFFE


  • Certification: If you are a Non-Financial Foreign Entity (NFFE), certify that you are actively engaged in a trade or business.   


Part XXX: Certification


  • Signature: Sign and date the form.

  • Printed Name: Print your name below the signature.
